This is a form I have created to organize all the details of each piece of content. It is easy to fill out this form as you edit the clip then have all the info for updating on each site you sell on. Then if you have others in the clip you can attach the copies of their ID's, release forms, etc. Since I can only upload JPEG here, please contact me after purchase and I can send you the link for the downloadable file.
